Continuity of care: recovery management program
One-year aftercare: Includes an evidence-based Recovery Management Program that will amalgamate the skills and treatment received during their stay. This program will include telephone check-in calls, group and individual sessions. Clients who are eligible will be paired with a Recovery Management Therapist who is best suited to support their needs.”
Holistic transition support: The program addresses professional reintegration, social stability, and resilience-building to sustain recovery beyond discharge. Program sessions include implementing healthy structure, habits, and boundaries, and Mindfulness-Based Relapse Prevention.
